Instruction for Submission:
This form should only be submitted by devolved Welsh authorities to notify the Wales Procurement Review Unit (WPRU) of the exclusion, replacement, or removal of a supplier from a public sector procurement, in accordance with section 59 of the Procurement Act 2023.
